Appointment request:

Please call the office 919-998-9177 to schedule an appointment.  As I am spending most of my time with clients, I am often not able to answer the phone.  Please leave a message with your name, date of birth and phone number, and I will return your call.  

Here is also a link to my Profile on  Patient Fusion.  This allows established clients to request a followup appointment.  If you are interested in becoming a new client, you may use the system as well, but please understand that most of the available appointment times listed are for short followup visits and not available for an initial visit.  I am allowing up to 90 minutes for the first visit.  Feel free to request an appointment and enter your contact information, and I will call you with available appointment times.   If you don’t hear back from me within 2 business days, please call in addition. 

As an established client, you also have access to your personal Patient Portal.  An invitation and link to the to your portal is typically emailed after your first visit.  Please call if you need assistance or cannot find the invitation email.
